In-Home Rehab

Improve your ability and functioning with our in-home rehab programs. Our experienced care team helps you stay independent and safe in your own home and decreases your risk of unexpected visits to the hospital. Therapy teams include Physical Therapy (PT), Occupational Therapy (OT), Speech Language Pathology (SLP), Respiratory Therapy (RT), and Registered Dietitian Services. All are tailored according to individual needs and your Doctor’s orders. If you started an inpatient rehab program, such as PT or OT, completing and continuing them at home maximizes results.

Physical Therapy

Physical Therapists (PT) help you to strengthen weak muscles, regain balance and coordination, ease pain, increase endurance, improve mobility, and prevent other parts of your body from being injured while compensating for weak areas. Our PTs accomplish this by creating personalized home fitness and wellness programs that address disease or injury through exercise, therapeutic activities, manual therapy, and education.

Occupational Therapy

Occupational Therapists (OT) help you adapt and modify items in your environment and teach you how to perform tasks in a way to be more independent and safer in your activities of daily living. Activities of daily living include eating, cooking, moving to different areas of your home, dressing, bathing, and toileting. Our OTs provide sensory motor therapeutic activities for improving range of motion, coordination, strength, endurance, and balance to assist your daily life skills. Our OTs also make recommendations related to adaptive devices or equipment. Your home is the ideal setting for functional training.
